Renewal Application For General Membership (Peninsula)

Yes, I wish to renew my membership of the Qualified Gas Contractors Association. I will live up to and support the CODE OF ETHICS and BYLAWS of the association.

Terms And Conditions

1. The applicant acknowledges that the council, at its discretion, may make such independent inquiries as it deems appropriate with licensing bureaus, state and municipal authorities, credit agencies, DPOR, consumer protection boards and similar organizations regarding the qualifications of the applicant for membership in the association
2. The applicant further acknowledges and accepts that the part of the determination as to his or her qualification for membership is dependent at least in part upon subjective analyses and conclusions by the council.
3. The provision by the applicant of any false and/or misleading information in this Application shall constitute automatic grounds for denial of his application and furthermore as grounds for suspension or revocation of membership if the applicant has been accepted prior to the discovery of such false and/or misleading information.
4. The applicant has read the Code of Ethics (a copy of which has been included) and agrees to abide by them.
5. The applicant acknowledges that membership in QGC is limited to those businesses that meet QGC’s standards of excellence. The applicant further acknowledges that (1) active non provisional membership provides the exclusive right to use and display the QGC logo; (2) the public has come to rely on the QGC logo as a statement of quality and that businesses displaying the QGC logo are in fact are in fact members in good standing of QGC; (3) display of the QGC logo by a business that is not a member in good standing constitutes a false and misleading business practice that harms the public and is detrimental to those businesses that are in good standing of QGC; (4) strict enforcement by QGC to ensure that only members in good standing are displaying and using the QGC logo is a desirable public service and an important benefit to members in good standing. The applicant will display and use the QGC logo only as permitted by QGC by laws that may be changed from time to time by the governing council and (2) the applicant will diligently monitor the use of the QGC logo by others and will report any miss use of the QGC logo to the governing council promptly. Once membership is terminated, regardless of the cause of termination, the applicant warrants and represents that it shall (1) immediately discontinue all use and display of the QGC logo by and for itself or any person employed by the applicant; (2) take all appropriate steps to ensure that the public is not mislead or misinformed about the discontinuation of membership; (3) pay all of QGC’s costs and fees, including but not limited to attorney’s fees, that QGC may incur in enforcing this provision; and (4) consent to the entering of consent orders or injunctions that are reasonably necessary to protect the interests of the public and of the membership in good standing of QGC in addition to paying damages for the harm caused.
